Our well-established healthcare facility in Albany is seeking a skilled Nurse Practitioner to provide comprehensive, patient-centered care. This role focuses on clinical oversight, patient management, and collaboration with interdisciplinary teams in a structured weekday setting.

Key Responsibilities:

Manage patient admissions and discharges
Oversee medication management and treatment plans
Evaluate changes in patient condition and adjust care accordingly
Perform wound care assessments and coordinate referrals
Maintain accurate and timely patient documentation
Collaborate with physicians and interdisciplinary teams to support outcomes
Ensure compliance with clinical standards and regulations

Qualifications:

Active Nurse Practitioner license in New York
Minimum 2 years of experience in hospital or subacute rehabilitation settings
Strong clinical assessment and decision-making skills
Excellent communication and organizational abilities
