Overview:
Our specialized nephrology practice is seeking a Nurse Practitioner to provide high-quality care for patients with chronic kidney disease. New graduates are welcomed to apply. This role offers the opportunity to work in a collaborative, low patient-volume environment focused on delivering personalized care and strong clinical outcomes.

Key Responsibilities:

• Evaluate and manage patients with chronic kidney disease
• Conduct patient rounds and monitor treatment progress
• Perform comprehensive patient assessments and medical histories
• Develop and implement treatment plans in collaboration with physicians
• Provide patient and family education on disease management
• Coordinate care with interdisciplinary healthcare teams
• Maintain accurate and timely medical documentation
• Support continuity of care and positive patient outcomes

Qualifications:

• Master's or Doctoral degree from an accredited Nurse Practitioner program; active California NP license required
• National NP certification (e.g., FNP, AGNP) preferred
• Strong clinical, communication, and patient care skills, with the ability to work effectively within a multidisciplinary team
