Nurse Practitioner (NP) – Nephrology
Santa Maria, CA
Overview:
Our practice in Santa Maria is seeking a Nurse Practitioner (NP) to evaluate and manage patients with chronic kidney disease. This role combines patient rounding, treatment planning, and collaboration with physicians and the interdisciplinary care team.

Key Responsibilities:
- Evaluate and manage patients with chronic kidney disease
- Conduct patient rounds and monitor treatment progress
- Perform comprehensive patient assessments and medical histories
- Develop and implement treatment plans in collaboration with physicians
- Provide patient and family education on disease management
- Coordinate care with interdisciplinary healthcare teams
- Maintain accurate and timely medical documentation
- Support continuity of care and positive patient outcomes
Qualifications:
- Master's or Doctoral degree from an accredited Nurse Practitioner program
- Active, current Nurse Practitioner (NP) licensure in California
- National NP certification (e.g., FNP, AGNP) preferred
- Strong clinical, communication, and patient care skills, with the ability to work effectively within a multidisciplinary team
