We are seeking a dedicated Nurse Practitioner to join a patient-focused practice in Chandler, AZ. This role offers a hybrid schedule with a mix of in-person and telehealth care, focusing on managing acute and chronic pain conditions.

Key Responsibilities:

Conduct patient evaluations and follow-up visits
Manage care for acute and chronic pain conditions
Develop and implement individualized treatment plans
Perform in-office procedures as appropriate
Monitor patient progress and adjust care plans
Maintain accurate and timely documentation
Collaborate with the clinical team to ensure quality care

Qualifications:

Active Arizona Nurse Practitioner license
Minimum of 2+ years of NP experience
Experience in pain management or related specialty preferred
Strong clinical assessment and patient management skills
Excellent communication and interpersonal abilities
