We are seeking an experienced Nurse Practitioner (NP) to join our team in Albany, NY. This full-time role offers a consistent weekday schedule with no nights or weekends, allowing you to focus on delivering high-quality patient care in a collaborative clinical environment.

Key Responsibilities
- Manage patient admissions and discharges
- Provide medication management and treatment oversight
- Evaluate and respond to changes in patient condition
- Conduct wound care evaluations and coordinate referrals when needed
- Maintain accurate and timely documentation of patient care
- Collaborate with physicians and care teams to support patient outcomes

Qualifications
- Active Nurse Practitioner license in New York
- Minimum 2 years of experience in a hospital or subacute rehabilitation setting
- Strong clinical assessment and decision-making skills
- Excellent communication and organizational abilities
