(Archived) Psychiatric Mental Health Nurse Practitioner (PMHNP) - remote, Phoenix AZ area

Are you looking for a new opportunity that is fully remote? We’re currently looking for a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide primary care to psychiatric and at-risk patients through virtual communication while working remotely. Must live in the general Phoenix, AZ area.

We’re an Arizona-based medical group consisting of 18 locations and over 90 medical providers, who care for more than 120,000 patients statewide.

We use a Collaborative Care Model (CoCM) team-based approach to mental health that involves collaboration between primary care providers, behavioral health specialists, and Psychiatric providers to deliver integrated and comprehensive care to individuals with mental health conditions. The goal of CoCM is to improve access to mental health services through community-based recourses, enhance patient outcomes, and provide more holistic and coordinated care.

Job Responsibilities:

• Provide Psychiatric support and consultations as a member of our Behavioral Health Collaborative Care Team.
• Virtually consult with patients to address Behavioral Health concerns.
• Utilize quality improvement data to enhance patient care.
• Meet weekly with CMO and Behavioral Health Care Manager Lead.
• Work directly with care managers and referring PCP’s assistants.

Job Qualifications:

• Bachelor’s degree in nursing followed by a master’s degree in Psychiatric or Mental Health Nursing from accredited school for professional nursing training.
• Possess strong understanding of societies and their impact on mental health; ability to solve problems, monitor and examine patients.
• At least 1 year of clinical work experience as Psychiatric Nurse Practitioner
• At least 1 year of EMR experience; eClinicalWorks preferred.
• Must be bilingual (Spanish)!
