Job Description

The Behavioral Health Advanced Practice Clinician (APC), CareMore Primary Care, will serve as the primary behavioral health clinician for a panel of complex patients, as part of an integrated clinical team addressing medical, behavioral, and social health issues. This individual will focus on assessing, diagnosing and managing patients with severe mental illness, substance abuse, and other behavioral health conditions while providing oversight and clinical guidance to the primary care team for behavioral health care.

  • Assess, diagnose, and treat individuals and families with behavioral health conditions, ranging from serious mental illness to substance abuse, or the potential for such conditions, using their full scope of therapeutic skills, including the prescription of medication and administration of psychotherapy, as appropriate.

Essential Duties & Responsibilities

  • Assess, evaluate, diagnose, and manage the behavioral health conditions of patients
  • Performs mental status examinations, biopsychosocial assessments, psychotherapy, and, when appropriate, prescribes and manages medications within prescribed guidelines and instructions of physician leadership
  • Order, interpret, and evaluate diagnostic tests to identify and assess patient’s clinical problems and health care needs
  • Record physical findings, and formulate a plan and prognosis, based on the patient’s condition
  • Discuss case with the medical and behavioral health physician leadership and other care team members(social work, therapists, care managers) to prepare and enact a comprehensive patient care plan
  • Submit the goals of individual patients for periodic review and evaluation by the physician leadership
  • Refer patients to physician leadership for consultation or specialized health resources for treatment
  • Maintain complete medical records, including history, physical exams, and telephone message summaries timely and accurately
  • Attend behavioral health and/or medical rounds at the care center, perform e-consults as needed, collaborate with care center teams on both medical and behavioral health aspects of care

Work Location & Travel Expectations

Based in local market, with local travel. Provides nighttime, weekday or some weekend and holiday call coverage as part of work schedule.

Minimum Qualifications

  • 1-2 years of experience as a Nurse Practitioner
  • 3-5 years practice as a behavioral health clinician strongly preferred
  • Current RN and Nurse Practitioner license (FNP/ANP/GNP) in good standing with the state in which you are practicing
  • Masters of Science in Nursing
  • Board Certified either with AANP or ANCC
  • Psychiatry Training and/or Psychiatric Mental Health Nurse Practitioner board certified preferred but not required.
  • Prescribing authority/DEA licensure
  • NPI billing number
  • Optional: Bilingual (Spanish or French)
