Must be flexible with schedule as position is active 24 hours 7 days a week This includes alternative shifts such as day, evening, or overnight, weekends, and holiday coverage. Shifts can be scheduled over 8 or 10 hours.

The Alignment Virtual Care Center is a collaborative approach to providing patients telehealth services 24 hours a day, 7 days a week. It is intended to provide support for Alignment Healthcare patients by being available to address any concern at any time. This program provides patients with medical and social support through virtual visit when they need it, with the goal of preventing unnecessary hospitalizations, health complications, and unmanaged disease progression that can occur when timely clinical interventions are not provided or are not accessible. The virtual care center program is offered to eligible patients at no cost to them.

In this role, APCs will conduct virtual visits when patient triaging calls are escalated up from the VCC RN. The nature of the virtual visits can vary based on the complexity of the patient concern and acuity level. Scheduled visits will also be added to the calendar to help meet the demand of comprehensive visits goals. APCs will work within an interdisciplinary team environment that includes RNs, social workers, health coaches, and coordinators to provide and support both the clinical and social aspects of care for these patients.

General Duties/Responsibilities:

(May include but are not limited to)

  • Conduct virtual visits on Alignment members.
  • The virtual visit comprehensive assessment includes:
    • Past medical history
    • Review of symptoms
    • Physical examination
    • Medication review
    • Cognitive/depression screenings
  • Provide patient education, emphasizing importance of consistent monitoring of health needs, and recommending community resources
  • Facilitate patient empowerment and quality of life by promoting educated, independent patient choice on all aspects of care when necessary
  • Matching care desired with care given while providing true, informed consent
  • Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment
  • Communicate findings in your assessment that will be used to generate a post-visit summary note to the PCP updating him/her and as well as identifying potential gaps in care. These findings can also be used to refer the patient to the Care Anywhere program.
  • Educate members on topics such as chronic conditions, self-care, sick-day plans, and medication and compliance
  • Participate in weekly interdisciplinary team meetings to discuss and develop the most appropriate care plans possible based on the needs of our members/patients.
  • Comply with all HIPAA regulations and maintain security of protected health information (PHI)

Minimum Experience:

  • Computer literacy/EMR proficiency
  • At least one (1) year prior clinical experience

Education/Licensure:

  • Active California, North Carolina, and Nevada state license as a Nurse Practitioner/Physician Assistant
  • Active Nurse Practitioner/Physician Assistant Board Certification
  • Medicare Part D Prescriber
NP (Nurse Practitioner):
  • Active RN License (Must, upon hire)
  • Active NP License (Must, upon hire)
  • Furnishing number (Must, upon hire)
  • NPI Number (Must, upon start)
  • DEA Number (Must, within 6 months of start)
  • Board Certification: AANP, ANCC (Must, within 6 months of start)
  • Valid BLS (Basic Life Support) (Must, upon start)
  • Valid ACLS (Advanced Cardiovascular Life Support (Preferred)

PA (Physician Assistant):

  • Active PA License (Must, upon hire)
  • NPI Number (Must, upon start)
  • DEA Number (Must, within 6 months of start)
  • Board Certification: NCCPA (Must, within 6 months of start)
  • Valid BLS (Basic Life Support) (Must, upon start)
  • Valid ACLS (Advanced Cardiovascular Life Support (Preferred)

Other:

  • Knowledge of current clinical standards of care
  • Ability to work independently
  • Experience in care of older adult (geriatric) patients preferred
  • Home care experience preferred
  • Excellent administrative, organizational and verbal skills preferred
  • Effective communication skills especially with older adults preferred
  • Previous EPIC experience preferred but not required preferred
  • Must be open to flexible schedule as position is active 24 hours a day, 7 days a week
