50335– Family Medicine & Urgent Care (NP/PA) Locums Role: Possible Locums to Permanent

· Coverage: Mon- Fri, 4x 10-hour days, filling in for leave.

· Practice: Sole Medical Provider

· Census: Adults and Children of all ages. Patients per day: 15+

· Scope: Includes the management of chronic conditions, annual exams, acute illnesses, urgent/emergent care. Treating or stabilizing to send home, or transport for higher level of care.

· EMR: Athena

Requirements of the Family Medicine & Urgent Care (NP/PA) Role:

· CO state license: active, unrestricted (in-hand).

· Board Certification – Family Nurse Practitioner

· Experience: 2-years

· DEA, BLS, ACLS, PALS

· CV (MM/YYYY, 30+day gaps explained – work and education)
