Position: LVN Case Manager
Location: Los Angeles, CA
Duration: 7 Weeks
Job Information
- The inpatient LVN case manager performs and documents initial and concurrent reviews for inpatient admissions; utilizes MCG criteria for review; coordinates discharge plans with coordinator; participates in weekly interdisciplinary rounds
Requirements
- Graduation from an accredited LVN nursing program
- Valid license as a Licensed Vocational Nurse
- Experience in and willingness to be part of multi-disciplinary team
- Experience with physically or mentally impaired adults and/or geriatric population
- 3 years experience in public health nursing, acute care, case management and/or in-home health care
- 2 years of managed care experience in case management with focus in inpatient and/or outpatient ambulatory care
- Bilingual in English/Spanish
