Seeking a full time RN Care Manager Supervisor to join a great team in Vermont!

• The ideal candidate will have 3-5 years care management experience and some supervisory experience mixed in there

• Position will oversee:

  2 FTE RN Care Manager (1 for PCU and 1 for ED)

  1 Social Work Care Manager (generally PCU)

  In total: 8 staff (inclusive of per diem)

JOB SUMMARY

The RN Care Manager Supervisor assesses appropriateness of intensity of services for patients, making
recommendations for change, as needed. Applies utilization review criteria to daily assessments of patients
needs. Supports medical staff in securing appropriate level of care for patients. Participates in discharge planning
process to ensure continuity of care in the most appropriate setting. Plans, organizes and directs the Case
Management Department including but not limited to, policies and procedures in accordance with the established
policies of the hospital and in accordance with all applicable laws and regulations, scheduling of staff, preparing
and reporting department budget. Coordinates with other departments to provide efficient and appropriate
services to patients and hospital staff.

QUALIFICATIONS

Education:
o Graduate of an accredited school of registered nursing with current licensure as a Registered Nurse in
the State of Vermont. Bachelor of Science Degree in Nursing required. Master’s Degree preferred.

Experience:

o Three years experience in a hospital setting required. Utilization management and/or case management
experience desired.

Special Skills:
o Certification in utilization management expected within 2 years. Excellent verbal and written
communication skills required.
