Job Description

The Opportunity:

-The Case Manager will plan, assess, implement, monitor, and evaluate the options and services required to meet the patient's health needs

-$45-$49 an hour

-$10,000-$12,500 relocation assistance and at least $5,000 sign-on bonus!

-Must have a BSN and 2 years minimum case management experience

-GREAT PAY AND INCREDIBLE BENEFITS PACKAGE!!

-Outpatient Clinic setting

-The Case Manager will communicate effectively with clients and make sure cost effective resources are being implemented

-Free 30 day housing upon start
