Registered Nurse – Community Health

We’re seeking a compassionate and skilled Registered Nurse (RN) to join a respected community health organization in Boston. In this role, you’ll work within a collaborative, mission-driven team to provide direct and indirect nursing care, patient education, triage, and care coordination in a diverse, dynamic environment.

Position Highlights:

• Schedule: Full time, some evenings and Saturdays required

• Setting: Community-based outpatient healthcare

Key Responsibilities:

• Assess and triage patients both in person and over the phone

• Collaborate with providers and care teams to ensure seamless patient care

• Manage medication refills, vaccinations, and routine procedures (EKGs, wound care, suture removal, etc.)

• Provide patient education to support self-management and wellness

• Maintain accurate, timely documentation in the electronic health record system

• Participate in team huddles and quality improvement initiatives

Requirements:

• RN license in the Commonwealth of Massachusetts

• Current CPR certification

• Strong critical thinking, organizational, and interpersonal skills

• Comfort working in a culturally diverse environment

• Bilingual (Spanish/Portuguese) preferred

• Experience with electronic health records required
