Universal Registered Nurse (RN)
Aurora, Colorado | Clinic & In-Home Care | $41.73 - $54.28/hour | Tues-Sat

Clinical Magnet is seeking a compassionate Universal Registered Nurse (RN) to join a leading healthcare organization in Aurora, Colorado. Position Overview
The Universal RN provides skilled nursing care in both the clinic and participants' homes while collaborating with providers and the interdisciplinary team to develop and implement individualized Plans of Care. This role includes comprehensive assessments, medication management, patient education, care coordination, quality improvement initiatives, and end-of-life support.

Responsibilities
  • Perform comprehensive nursing assessments, reassessments, and ongoing participant evaluations.
  • Develop, implement, and update individualized Plans of Care.
  • Administer medications, wound care, treatments, and other skilled nursing interventions.
  • Monitor patient conditions and communicate changes to providers promptly.
  • Coordinate specialty appointments, diagnostics, hospital admissions, and transportation as needed.
  • Educate patients and caregivers on medications, disease management, equipment, and safety.
  • Participate in interdisciplinary care meetings, family conferences, and case reviews.
  • Maintain timely and accurate electronic medical records and clinical documentation.
  • Assist with clinic operations, including exam room preparation and participant care.
  • Provide skilled nursing services in patients' homes, including preventive, restorative, and rehabilitative care.
  • Supervise LPNs, CNAs, and Home Health Aides according to regulatory requirements.
  • Support end-of-life care planning, patient advocacy, caregiver education, and bereavement resources.
  • Maintain compliance with HIPAA, OSHA, infection prevention standards, and all applicable regulations.
  • Participate in quality improvement initiatives and on-call rotation as assigned.

Qualifications
Required
  • Current, unrestricted Colorado Registered Nurse (RN) license.
  • Current BLS and First Aid certification through the American Heart Association or American Red Cross.
  • Minimum 1 year of experience caring for frail or elderly patients.
  • Valid driver's license, reliable transportation, and current auto insurance.
Preferred
  • Bachelor of Science in Nursing (BSN)
  • Gerontological Nursing Certification
  • Hospice or End-of-Life Care experience
  • Experience with geriatric assessments and care planning
  • Bilingual in Spanish, Russian, Punjabi, or Hmong
Travel
  • Up to 75% local travel throughout the Aurora service area.
