Registered Nurse Case Manager
Care Coordination | Geriatric Patient Population | Monday - Friday

Clinical Magnet, a division of ICON Medical Network, is looking for an experienced Registered Nurse Case Manager to join a collaborative PACE healthcare team in Orlando, Florida.

This is an excellent opportunity for an RN with experience in care coordination, discharge planning, utilization management, or geriatric care who enjoys working closely with an interdisciplinary team to help patients transition safely between hospital, skilled nursing, and home settings.

This is a DIRECT HIRE opportunity!

Responsibilities
  • Coordinate care for participants admitted to hospitals and skilled nursing facilities
    • Assess patient needs and develop individualized plans of care in collaboration with providers, families, and the interdisciplinary team
    • Participate in daily interdisciplinary team meetings and care planning
    • Coordinate discharge planning and transitions from acute and post-acute settings back to home or an appropriate level of care
    • Identify equipment, home care, appointments, and community resources needed following discharge
    • Monitor hospitalized participants and patients receiving skilled services in SNFs, providing daily updates to the care team
    • Review clinical information and participate in utilization management to ensure appropriate levels of care
    • Coordinate transfers to appropriate facilities and ensure required documentation is completed
    • Communicate effectively with patients, families, providers, facility staff, and community agencies
    • Provide patient and family education related to discharge planning and chronic or acute conditions
    • Maintain accurate medical records and required documentation
    • Develop relationships with hospital and skilled nursing facility staff involved in discharge planning
    • Visit the PACE center, hospitals, and contracted SNFs as needed to support care coordination and relationship development

Qualifications
Required:
  • Associate Degree in Nursing
    • Current Florida Registered Nurse license
    • Current BLS and First Aid certifications through the American Heart Association or American Red Cross prior to hire
Preferred:
  • 3+ years of experience with care coordination and discharge planning
    • 3+ years of healthcare experience, preferably with a geriatric population
    • Bachelor's Degree in Nursing
    • Bilingual skills
    • Gerontological Nursing certification

Compensation & Benefits
  • $80,700 - $105,000/year
  • Comprehensive benefits package
  • Monday-Friday, 9:00 AM-5:00 PM schedule

  • Approximately 25% local travel
