Clinical Magnet (part of ICON Medical Network), is partnering with a senior healthcare organization seeking a compassionate Home Health Registered Nurse to join their team in Philadelphia, Pennsylvania. This role offers a unique blend of autonomy and team collaboration, with the majority of your time spent providing care directly in patients' homes. Key Responsibilities:
- Provide skilled nursing care to patients in their homes, ensuring comfort, safety, and quality outcomes
- Assess, monitor, and document patient conditions, reporting changes and collaborating with providers on care plans
- Administer medications and treatments, while educating patients and families on proper use and compliance
- Coordinate care, including scheduling appointments, facilitating referrals, and supporting hospital transitions
- Complete clinical documentation, care plans, and required certifications/recertifications in a timely manner
- Participate in care conferences and collaborate with interdisciplinary teams (PT, OT, SLP, Social Work)
- Support and supervise LPNs and home health staff, ensuring high standards of care delivery
- Provide health education and counseling to patients and caregivers
What We're Looking For:
- Active, unencumbered registered nurse license within the state of Pennsylvania
- Associate's Degree in Nursing (or higher)
- BLS & First Aid certification (AHA or Red Cross)
- At least 1 year of experience working with geriatric or medically complex populations
- Valid driver's license, reliable transportation, and auto insurance
- Strong clinical judgment, communication skills, and a patient-centered mindset
