Job Description
Position: Case Manager RN
Shift: Evening – 12:00 PM to 8:30 PM (1 weekend per 4-week schedule)
Sign-On Bonus: $10,000
Overview:
Join us as a Case Manager RN and help guide patients through their care journey while ensuring quality outcomes and efficient use of resources. You will coordinate patient care, discharge planning, and utilization management while collaborating with medical providers, families, and interdisciplinary teams to optimize patient care and experience.
Key Responsibilities:

Conduct comprehensive assessments of medical and psychosocial needs

Develop and document individualized care plans

Lead interdisciplinary team discussions and care coordination activities

Monitor patient admissions for appropriateness and medical necessity

Facilitate patient discharge and follow-up services

Act as liaison between patients, families, providers, and third-party payers

Track trends, identify barriers to care, and recommend process improvements

Support high-risk and under-resourced patient populations

Maintain compliance with regulatory requirements and HCA policies
Requirements:
-
Minimum 3 years recent Case Manager experience in acute care (or 3 years combined acute care plus home health/insurance experience with 1 year acute care in last 5 years)

RN license in Florida or compact license with FL license within 90 days

Associate’s Degree in Nursing required; Bachelor’s preferred

Case Management, Nursing, or Utilization Review certification preferred

Experience in Med/Surg, Tele, Neuro, ICU, PCU, or ED units is also considered
