Registered Nurse (RN) Case Manager - Hybrid
A Registered Nurse (RN) Case Manager is needed in Lynn/Beverly, MA.

The Position
We are seeking a full-time Registered Nurse (RN) to serve as a Case Manager, helping patients set up services from their homes, and overseeing patient care through individualized care plans in the Lynn and Beverly, MA area. This role is non-clinical and does not involve direct patient care or bedside nursing.

- Salary- $75,000- $95,000+ (Based on Experience)

Responsibilities
- This Position Does Not Require Direct/ Bedside Patient Care
- Hybrid, Monday- Friday Schedule, 8:00am- 4:00pm
- 3 Days in the Community Seeing Patients between the Lynn, Beverly and surrounding areas.
- Developing individualized Plans of Care, assisting patients with setting up necessary services, and providing ongoing follow-up to ensure continuity and effectiveness of care.
- 2 Days at Home for Administrative Work/ Follow Up with the Care Team
- Works With Interdisciplinary Team
- No On Call Responsibilities
- No Nights
- No Weekends
Requirements
- MA Registered Nurse Licensed
- 2+ Years of Experience as a Registered Nurse
- 2+ Years of Home Care Experience Preferred, But Not Required
- Must Have Great Communication Skills
- CPR Certified
- Covid Vaccination Required or Exemption
