Registered Nurse (RN) Case Manager
A Registered Nurse (RN) Case Manager is needed in Clewiston, FL.

Salary and Benefits
- The salary range is $75,000 to $95,000 per year
- $600 Monthly Fuel Stipend if 40 Miles from Facility

Responsibilities
- Schedule: Monday-Friday (8am – 5pm)
- Caseload: 8-13 Patients per Day
- Call: 1 in 6 or 7 Weeks
- Provide quality care through implementation and evaluation of individualized patient care plans, discharge planning and complex outpatient needs.
- Perform prospective, concurrent and retrospective evaluation of clinical information to establish medical necessity, appropriateness of services and cost effectiveness.
- Serve as patient and family advocate during hospital stay and outpatient status.
- Conduct in-home or hospital evaluations in some circumstances.
Requirements
- Associate’s Degree in Nursing
- Registered Nurse License in the State of Florida
- Certification in Case Management (or the ability to obtain within one year of employment)
- Minimum of 2 years of experience as a Medical Case Manager
- Current BLS certification
