Job Title: Nurse Case Manager - Hospice

As a Hospice Nurse Case Manager, you will oversee a defined caseload, guiding care from admission through end-of-life, while working within a supportive interdisciplinary team focused on dignity, comfort, and family-centered support.

Key Responsibilities:

Manage assigned hospice caseload and coordinate all aspects of care from admission to discharge or end-of-life
Conduct comprehensive initial and ongoing nursing assessments
Develop, implement, and update individualized care plans based on patient and family goals
Collaborate with physicians on symptom and pain management, including medication adjustments
Coordinate care with CNAs, social workers, chaplains, and other team members to ensure holistic support
Serve as the primary clinical contact for patients and families
Maintain accurate documentation and compliance with hospice standards

Qualifications:

RN with at least 1 year of hospice or home health experience, or strong interest in hospice care
Strong clinical assessment, care planning, and symptom management skills
Excellent communication, teamwork, and patient-centered approach
Compassionate and dedicated to providing end-of-life care
