Job Title: Registered Nurse (RN) - Hospice Case Manager
Location: Longmont, CO

Overview:
Provide compassionate, patient-centered hospice care in Longmont by managing your own caseload and leading an interdisciplinary team. Deliver care across private homes and facility settings while supporting patients’ comfort and dignity.

Key Responsibilities:
- Manage assigned hospice caseload as primary RN contact
- Perform assessments and develop individualized care plans
- Update care plans regularly in collaboration with the team
- Coordinate with physicians, CNAs, social workers, and chaplains
- Provide care across private homes and facility settings
- Ensure continuity of care and patient-family support
Qualifications:
- Active Colorado RN license
- Minimum 2 years of hospice experience
- Reliable vehicle for travel to patient locations
- Strong assessment, care planning, and coordination skills
