We’re hiring a Registered Nurse (RN) Case Manager in Lynn, Massachusetts who wants autonomy, a flexible hybrid schedule, and meaningful impact with older adults without bedside demands.

You’ll step into a community-based role focused on developing individualized plans of care, coordinating services, and ensuring continuity for patients living at home. This position does not require direct bedside care. You’ll spend three days in the community meeting patients and two days working remotely on care coordination and follow-up.

You’ll practice in Lynn, Massachusetts, a coastal city just north of Boston offering diverse communities and convenient access to both urban and seaside living.

Compensation & Benefits
• Salary ranging from $84,000 to $108,000 based on experience
