Our home health team is seeking a Registered Nurse Case Manager to deliver personalized care in patients’ homes. This role focuses on managing care plans, coordinating services, and supporting patients and families to achieve optimal outcomes in a structured weekday schedule.

Key Responsibilities:

Conduct patient assessments, admissions, and OASIS documentation
Develop, implement, and update individualized care plans
Manage recertifications, resumptions of care, and discharges
Coordinate care with physicians and interdisciplinary team members
Educate patients and families on conditions and care plans
Perform supervisory visits and monitor patient progress
Maintain accurate documentation and ensure regulatory compliance
Support quality care and patient outcomes through effective case management

Qualifications:

Active Registered Nurse (RN) license in Texas
Minimum 1 year of home health or hospice experience
Strong clinical assessment and care coordination skills
Excellent communication and organizational abilities
Ability to work independently in a home-based setting
