Overview:
Our Registered Nurse (RN) Hospice Case Manager provides comprehensive care coordination for hospice patients, ensuring high-quality, comfort-focused care across all stages of service. This role is responsible for clinical assessments, care planning, and interdisciplinary collaboration to support patients and their families.

Key Responsibilities:

Perform comprehensive patient assessments and ongoing clinical evaluations
Manage medication refills and coordinate care needs
Participate in interdisciplinary group (IDG) meetings
Oversee CNA visits and provide clinical supervision during care
Support limited admission duties primarily coordinated by clinical liaisons
Develop and maintain individualized hospice care plans
Ensure continuity of care aligned with comfort-focused goals

Qualifications:

Active Registered Nurse (RN) license
Minimum 1 year of hospice nursing experience
Strong clinical assessment and care coordination skills
Ability to work independently in a field-based setting
Effective communication and interdisciplinary teamwork skills
