Registered Nurse (RN) Case Manager
Denton, TX
Overview:
Our home health agency in Denton is seeking a Registered Nurse (RN) Case Manager to provide comprehensive patient care, care coordination, and documentation compliance in patients' homes. This role combines clinical assessment with care planning to support positive patient outcomes.
Position Offers:
- Schedule: Monday–Friday, 8:00 AM–5:00 PM
- Competitive salary: $85,000–$89,000 annually (DOE)

- Mileage reimbursement at $0.70/mile
- Additional on-call pay of $250 for every other weekend
Key Responsibilities:
- Conduct comprehensive patient assessments
- Develop and execute individualized care plans
- Coordinate care across the healthcare team
- Provide patient education
- Maintain accurate documentation and ensure compliance
- Perform basic wound care
Qualifications:
- Minimum 1 year of Home Health RN Case Management experience, with Oasis and EMR proficiency
- Current Texas RN license
- Valid driver's license and auto insurance
- Must reside in the Lewisville/Denton, TX area
