Registered Nurse (RN) – Hospice
Dallas, TX

Overview:
Our hospice care team in Dallas is seeking a Registered Nurse (RN) to provide comprehensive, compassionate care across facility and home settings. This full-time role offers a consistent Monday–Friday schedule within a collaborative interdisciplinary team.

Position Offers:

  • Schedule: Monday–Friday
  • Competitive salary: $43.00–46.00 per hour (DOE)

Key Responsibilities:

  • Conduct comprehensive patient assessments and full-body evaluations in facility and home settings
  • Manage medication refills and coordinate symptom management needs
  • Participate in Interdisciplinary Group (IDG) meetings and contribute to care coordination decisions
  • Develop, implement, and maintain individualized hospice care plans
  • Supervise and oversee CNAs during patient visits
  • Collaborate with the interdisciplinary hospice team to ensure continuity of care
  • Provide patient and family education regarding hospice services and end-of-life care
  • Support occasional admissions rotation (minimal, as most admissions and GIP cases are handled by Clinical Liaisons)

Qualifications:

  • Active Registered Nurse (RN) licensure in Texas
  • Minimum 1 year of hospice nursing experience
  • Strong clinical assessment and care coordination skills
  • Ability to manage a field-based caseload independently
  • Valid driver's license with reliable transportation
