Registered Nurse (RN) – Case Manager
North Chicago, IL

Overview:
Our hospice organization in North Chicago is seeking a Registered Nurse (RN) Case Manager to provide comprehensive nursing care to patients with life-limiting illnesses in their homes and community settings. New grads are welcomed to apply for this full-time role within a collaborative interdisciplinary team.

Position Offers:

Key Responsibilities:

  • Conduct comprehensive nursing assessments of patients' physical, emotional, and psychosocial status in home and community settings
  • Develop, implement, and evaluate individualized hospice care plans focused on symptom control, comfort, and quality of life
  • Provide expert pain and symptom management, including medication administration, monitoring, and timely intervention
  • Educate patients and families on disease progression, care options, medications, and what to expect at each stage of care
  • Coordinate care and communicate regularly with physicians, social workers, chaplains, aides, and other team members
  • Perform home safety assessments and implement interventions to reduce risk and promote comfort and caregiver safety
  • Document all assessments, interventions, and patient responses accurately and promptly in the EHR
  • Participate in interdisciplinary team meetings, case conferences, and on-call rotations to support continuous patient care

Qualifications:

  • Active, current Registered Nurse (RN) licensure in Illinois
  • Prior experience in hospice case management, including full-cycle patient care, preferred
  • Certification in Hospice and Palliative Nursing (CHPN), or willingness to pursue, preferred
  • Valid driver's license and reliable transportation required
