Registered Nurse RN / Case Manager / Home Health

Job Responsibilities

As a Case Manager RN you will provide a wide range of nursing care to patients in the home care setting.

Additional responsibilities for the Case Manager RN include:

Flexible Schedule
Focus primarily on patient assessment, quality care, teaching of patients and caregivers, and performance of skilled procedures.
Deliver care based on Standards of Practice for the particular state of practice and must be in compliance with company policy and regulatory requirements.

Job Requirements

We are looking for dedicated and professional Case Manager RN who are committed to providing exceptional patient care and service. It is also important that you are highly organized and that you demonstrate excellent communication and interpersonal skills.

Specific qualifications for the Case Manager RN role include:

Must be a licensed Registered Nurse (RN) in the state of Missouri.
A minimum of one year experience as a Registered Nurse is required.
Previous experience in the home care setting is desired.
Possess the ability to transfer, maneuver, and lift incapacitated clients in the home setting.
