$60,000 - $65,000 Starting Annual Salary, based on experience

Registered Nurse RN / Case Manager / Home Health




Job Responsibilities: The Registered Nurse RN Case Manager will be responsible to manage clinical workflow to include start-of-care reports, physician order review, and documentation quality review. Additional responsibilities for the Registered Nurse RN Case Manager include but are not limited to:


Completing assessments to determine nursing needs
Developing a plan-of-care
Administering Medications
Counseling the patient and family in meeting nursing-related needs
Flexibility in traveling to Citrus County
Must be willing to travel within Sumter & Lake County


Job Requirements: We are looking for dedicated and professional Registered Nurse RN Case Manager who is committed to providing exceptional patient care and service. It is also important that you are highly organized and that you demonstrate excellent communication and interpersonal skills. Specific qualifications for the Registered Nurse RN Case Manager role include:


Must be a Registered Nurse in good standing licensed by the state of Florida.
Must have a degree from an accredited school of Nursing.
Must have one year of experience as a Registered Nurse
Home Health experience as a Registered Nurse required
Must be Medicare Certified
Must be able to relate positively and favorably to patients and family members.
Must be able to lift or transfer patients, medical supplies, and equipment.
