Position Overview:
The purpose of this position is to promote the advancement of coordinated, cost-effective, quality patient care and services within the acute care hospital in partnership with the health system care coordination strategy. Provide oversight and direction of the care coordination department functions for Deaconess Hospital, Inc. This will involve working closely with Department Managers across nursing units to integrate unit-based case management and social work functions with the unit care delivery system benefiting care coordination. Collaborates with nurse leaders at all levels to promote evidence-based practice and standards of care across nursing units. In coordination with the Business Manager provides financial management of Laboratory Services:
Position Qualifications:
- Minimum of 10 years in case management
- 5 years in leadership.
- Baccalaureate degree in nursing is required.
- Master’s degree in nursing or management/business required.
- Maintain competency and continuing education in area of specialization and in professional practice required.
- Demonstrated project management skills using six sigma methodology required. Knowledge of EPIC electronic health record preferred
