Job Description
We are hiring an experienced RN case manager for our specialty hospital in the Phoenix, AZ! Our center performs a broad range of outpatient surgical procedures. Our mission is to care for every patient and their family as if they were our own. Each patient, each family, each and every time.
Schedule: Full time Monday - Friday with an occasional Saturday. Light call with call pay differential.
Position overview: The position is responsible for facilitating the patient’s hospitalization from pre admission through discharge from the inpatient unit to the community or to an alternative level of care. The case manager interfaces with physicians, nurses, social workers, and other healthcare team members to expedite medically appropriate cost-effective care

Requirements: BSN preferred; Case Management Certification preferred; 5 years of recent Case Management Experience Required.
