Schedules: The RN Case Manager is Days (4x10s) RN Care Coordinator (5x8s)

Position overviews:

  • The RN Case Manager is responsible for facilitating the patient’s hospitalization from pre admission through discharge from the ED to the community or to an alternative level of care.
  • The RN Care Coordinator is responsible for ensuring that utilization of resources including medical services, length of stay and appropriate status class.

Requirements: Five years of acute hospital case management/care coordinator experience

License/Certifications: CA RN license; Preferred Case Management Certified
