Job Title: Registered Nurse (RN) – MDS Coordinator

Overview:
We are seeking a detail-oriented Registered Nurse to serve as an MDS Coordinator across our Boston locations. In this role, you will oversee MDS and RAI processes, ensure accurate documentation, and support compliance while collaborating with care teams to optimize resident outcomes and reimbursement.

Key Responsibilities:
Oversee and manage the MDS and RAI process for accuracy and timeliness
Maintain complete and compliant resident documentation
Monitor resident status and coordinate individualized care planning
Support Medicare and Medicaid reimbursement processes
Ensure adherence to regulatory and compliance standards
Collaborate with interdisciplinary teams to enhance care quality
Identify and implement improvements in documentation and workflow efficiency
Qualifications:
Active Massachusetts Registered Nurse (RN) license
Minimum 1 year of MDS experience
Strong organizational and time management skills
Effective communication and teamwork abilities
