Director Case Management – Full-Time Days

Shift Details
Full-time, day shifts with 7–10 hour schedules
Rotating weekends
Shift differentials offered for nights and/or weekends

Compensation and Benefits
Competitive compensation commensurate with experience
Up to $10,000 sign-on bonus
Relocation assistance considered on a case-by-case basis
Incentive bonus of 17.5%

Your Role

• Direct and evaluate departmental operations including staffing, competencies, and technology use

• Oversee case management core functions, ensuring quality, compliance, and timely care coordination

• Develop and maintain strong relationships with physicians, post-acute community partners, and hospital leadership

• Monitor performance metrics, implement improvement initiatives, and manage multiple priorities

• Lead a team of managers, supervisors, and administrative staff to achieve organizational goals
