Job Responsibilities

The Director of Case Management provides leadership and accountability for all activities pertaining to Case Management services. Additional responsibilities include but are not limited to:


11 Direct Reports( 8 Case Managers, 2 Social Workers and 1 Assistant)
Reports to the CFO
Responsible for reducing Length of Stay and Re-admissions
Responsible for Improving Hospital Discharge Time


Job Requirements

We are looking for a dedicated and professional Director of Case Management who is committed to providing exceptional patient care and service. It is also important that you are highly organized and that you demonstrate excellent communication and interpersonal skills. Specific qualifications for the Director of Case Management role include:


7 years of Case Management experience - Required
2-3 years of leadership experience in a Case Management setting - Required
Bachelors of Nursing-Preferred
Registered Nurse in the State of Florida-Required before starting
