Overview
POSITION SUMMARY: The Nurse Navigator, Cerebrovascular is accountable for care coordination from admission through care continuum for patient diagnosed with an acute ischemic/hemorrhagic stroke, subarachnoid hemorrhage and/or transient ischemic attack (TIA). For each assigned patient, the Nurse Navigator, Cerebrovascular functions in concert with the stroke team, case manager(s) and other members of the interdisciplinary care team and physicians to aid patients through the continuum of care and achieve optimal patient outcomes. The Nurse Navigator, Cerebrovascular acts as a liaison between hospital inpatient departments and ambulatory offices as a single point of contact and supports the collection and analysis of data related to patient outcomes and effective use and access to standardized resources to support evidence-based delivery of care.
Qualifications
REQUIRED QUALIFICATIONS:
Education: Bachelors Nursing
Licenses/Certifications: RN-Registered Nurse
Other Licenses/Certifications: SCRN certification required within two years of hire date.
Experience: Three years neuroscience nursing experience required and extensive knowledge of the care and treatment of the stroke patient throughout the care continuum
Other skills/knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem solving skills, with the ability to be detail oriented.
PREFERRED QUALIFICATIONS: CNRN certification preferred Other requirements/information to be added Knowledge of related admission orders, guidelines, treatment plan, testing, medications, discharge planning and ambulatory follow up of the patient populations Prior knowledge and participation in community events and resources
