Registered Nurse (RN)/Licensed Practical Nurse (LPN)

Summary:
We are seeking a dedicated and skilled RN/LPN to join our team as a Care Navigator at our independent living community in Mission, KS. The ideal candidate will provide nursing duties, work directly with patients for urgent and preventative care, and demonstrate strong phlebotomy skills. Prior experience in geriatrics is a plus but not required.

Responsibilities:
- Serve as a Care Navigator, assisting residents with understanding and accessing healthcare services.
- Perform nursing duties including assessments, care planning, and monitoring health conditions.
- Provide urgent and preventative care to residents, addressing immediate health needs.
- Conduct phlebotomy procedures with proficiency and accuracy.
- Educate residents on health maintenance and disease prevention.
- Collaborate with healthcare providers, families, and community resources.
- Maintain accurate and confidential patient records.
- Work independently, managing time effectively and prioritizing tasks.
- Ensure compliance with healthcare regulations and standards.
- Participate in community meetings and health education programs.

Qualifications:
- Current RN/LPN license in the state of Kansas.
- Strong phlebotomy skills.
- Ability to work independently and demonstrate strong organizational skills.
- Excellent communication and interpersonal skills.
- Ability to manage multiple tasks and prioritize effectively.
- Prior experience in geriatrics is preferred, but not required.
- Basic computer skills for electronic health record management.
