Qualifications:

•Degree/Education (5.2.4): Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE).
•Experience: The nurse shall have at least 12 months of experience within the last 48 months. Experience in a primary care clinic setting desirable.
•Licensure/Registration: Current, full, active, and unrestricted license to practice as a Registered Nurse as required in the TO. Have and maintain Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS).

•Knowledge: The nurse shall possess a working knowledge of principles and practices of ambulatory nursing care. Shall be able to fulfill all requirements of specialty Description/Performance Standards

Duties:

•Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost-effectiveness.
•Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF.
•Perform physical exam and health histories.
•Provide health promotions, counseling, and education.
•Administer medications, wound care, and numerous other personalized interventions.
•Conduct in-person and phone triages with patients regarding their use of and adherence to prescribed medications including psychopharmacological agents
used in patient treatment and consults with prescribing providers on-staff regarding concerns noted, refills required, and other issues identified during patient triage.
•Direct and supervise care provide by other healthcare professionals.
•Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel.
•Must have the ability to establish a nursing plan of care that effectively applies core disease management principles to mental health conditions and includes in that plan: a) assessment and identification of target populations; b)implementation and execution of Clinical Practice Guidelines or standards; c)collaboration and coordination of activities with other healthcare members; d)promotion and education of patient self-management actions.
•Recognize adverse signs and symptoms and quickly react in emergency situations.
•Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.
•Make referral appointments and arrange specialty care as appropriate.
•Assists with patient care related duties within their scope of practice as deemed necessary by the mental health flight commander.
•Perform nursing services identified in the TO.
•Conduct research in support of improved practice and patient outcomes.
•Skill in setting up, operating and monitoring specialized medical equipment such as cardiac monitoring devices, EKG units, oxygen analyzers, nebulizers, and other clinic specific equipment.
•Performs/assists with procedures such as bladder catheterizations, administration of oxygen, visual acuity tests, EKGs, irrigation of eyes and ears, tympanogram, splinting, phlebotomy and other clinic specific procedures, in accordance with established protocols.
•Participates in the orientation and training of newly assigned personnel, as appropriate.
•May be required to work various shifts or augment clinical nursing support during the absence of other team nurses due to illness, leave or deployment,
•Ensures a safe work environment, employee safe work habits and patient safety regulatory agencies, infection control policies, and process improvement initiatives.
•Responsible for the care of selected military members (DD2992 holders for all services) along with select specialties such as Fire Fighters, Explosive OrdinanceDisposal personnel, Personnel Reliability Program, and their dependents.
•Follows standards and ensures the quality of care to all empaneled patients. Provides care within ethical and legal boundaries. Applies the nursing process and collaborates with providers and consultants for the benefit of patient and organization.
•Adheres to the Accreditation Association for Ambulatory Health Care standards and Health Services Inspection requirements.
•Demonstrates concern for the cultural, psychosocial, spiritual and emotional needs of the patient. Guides and counsels paraprofessionals and other personnel they supervise.
•Promotes quality improvement, infection control, and safety standards/initiatives. Develops and monitors tools and metrics to evaluate and continuously improve the effectiveness of programs and the health of the defined population.
•Participates in disease and case management for the defined population.
•Uses population health guidelines to ensure patients receive recommended clinical preventive services.
•Monitors AF1041 log and coordinate appropriate follow-up care for patients.
