Job ID: 135202 Direct Hire Registered Nurse Role $44.66 to $58.08 per hour in Sacramento, California Clinical Magnet (part of ICON Medical) is looking for a compassionate Eligibility RN to join their In-Home Services team in Sacramento, California. This is a DIRECT HIRE, full-time opportunity, onsite.

Responsibilities of the Eligibility RN:

Schedule and conduct home visits to assess prospective participants and determine level of care. Complete documentation in collaboration with primary care physicians and the enrollment team. Participate in outreach programs to support community engagement. Serve as a member of the Interdisciplinary Team (IDT), coordinating additional assessments as needed. Provide consultation to case managers and staff on medical and nursing issues. Maintain knowledge of Medicare, Medicaid, and PACE program eligibility guidelines. Assist with in-home assessments, reassessments, supervisory visits, and routine RN care as scheduled.

Qualifications of the Eligibility RN:

Associate degree in Nursing from an accredited school. Current RN license in the state of practice. Current First Aid and BLS certifications (American Heart Association or Red Cross). 3+ years of healthcare experience with emphasis in geriatrics. 1+ year working with frail or elderly populations.

Compensation and Benefits for Eligibility RN:

$44.66 - $58.08 per hour (based on experience). Full-time, onsite schedule. Comprehensive benefits package.
