Overview:
We are seeking a compassionate Hospice RN to provide skilled nursing care to patients in their homes or care facilities. This role involves developing individualized care plans, monitoring patient conditions, coordinating with the healthcare team, and supporting patients and families through end-of-life care.

Key Responsibilities:

Provide compassionate nursing care to patients in home or facility settings
Develop and manage individualized care plans
Monitor patient conditions and report changes promptly
Educate patients and families about care and expectations
Coordinate with physicians, hospice staff, and other healthcare providers
Arrange necessary services or referrals
Maintain accurate and timely patient records
Offer emotional support to patients and their families

Qualifications:

Active RN license in Texas
Minimum 2 years of hospice experience
Strong clinical assessment and patient care skills
Excellent communication and interpersonal abilities
Ability to work independently and collaboratively
Compassionate, patient-focused approach
