MDS Coordinator – Skilled Nursing Facility | Worcester & Milford, MA | $44–$46+ per hour

A skilled nursing facility serving Worcester and Milford, Massachusetts, is seeking an experienced MDS Coordinator to join its team. Key Responsibilities

• Complete and submit accurate, timely MDS assessments in compliance with CMS/RAI guidelines
• Coordinate the interdisciplinary team to ensure assessments, care plans, and documentation are completed on schedule
• Develop, review, and update resident care plans based on assessment findings
• Monitor documentation to support accurate PDPM classification and reimbursement
• Track assessment schedules and submission deadlines
• Stay current on federal and state regulatory requirements related to MDS

Qualifications

• Minimum of 1 year of MDS experience required
• Strong knowledge of the RAI process, PDPM, and CMS regulations
• Excellent organizational skills and attention to detail
• Ability to collaborate effectively with nursing, therapy, and administrative staff
