You’ll step into a nursing role providing care to seniors in outpatient and community-based settings. You’ll conduct assessments, coordinate individualized care plans, manage medications, and help older adults remain safely and independently in their homes. You’ll enjoy clinical autonomy while collaborating with a supportive interdisciplinary healthcare team.

• You hold an active Massachusetts Registered Nurse (RN) license
• You have at least 2 years of experience caring for geriatric patients
• You’re comfortable conducting nursing assessments and developing individualized care plans
• You have experience with medication management and care coordination
• You’re willing to travel locally using your personal vehicle
• You have strong clinical judgment, communication, and teamwork skills
• You value compassionate, patient-centered care and helping seniors maintain their independence

Compensation & Benefits
• Annual salary: $80,000–$104,000
