We are seeking a dedicated and compassionate Home Health Registered Nurse (RN) to join our team in Waco, TX. As a Home Health RN, you will play a vital role in assessing patient needs, developing individualized care plans, and delivering skilled nursing services in the comfort of patients’ homes.

Job Responsibilities:
- Complete Start of Care (SOC) and OASIS assessments in accordance with CMS and agency guidelines
- Perform recertification and resumption of care visits
- Conduct wound care and supervisory visits as assigned
- Accurately document all visits and assessments using Homecare Homebase (EMR)
- Coordinate care with physicians, therapists, and interdisciplinary team members
- Educate patients and caregivers on treatment plans and disease management
- Ensure compliance with agency policies, state regulations, and infection control standards
- Participate in on-call rotation (1 week per month)

Qualifications:
- Active and valid Registered Nurse (RN) license in the state of Texas
- 1+ years of home health nursing experience
- Proven experience with Start of Care (SOC) and OASIS assessments
- Familiarity with Homecare Homebase EMR required
- Valid driver’s license and ability to operate a company vehicle
