RN / Registered Nurse - Home Health

NEW Registered Nurse (RN) – Home Health | Direct Hire

Seeking a compassionate and experienced Registered Nurse to provide skilled home health nursing care. This role focuses on patient assessment, care planning, medication management, coordination with physicians and care teams, and patient/family education in the home setting.

Key Responsibilities:

• Assess patient eligibility and needs for home health services

• Develop, implement, and revise individualized plans of care

• Provide skilled nursing care considering physical, emotional, cultural, and social factors

• Coordinate care with physicians, therapists, aides, and interdisciplinary teams

• Perform medication reconciliation and identify potential drug interactions or compliance issues

• Educate patients and caregivers on disease management, safety, and emergency care

• Complete accurate and timely documentation in the EMR

• Conduct Home Health Aide supervisory visits every 14 days

• Support quality assurance, PDGM case conferencing, and performance improvement initiatives

• Ensure infection control and agency policies are followed

Qualifications:

• Active RN license (multi-state or state-specific)

• Home Health nursing experience required

• Strong clinical judgment, communication, and organizational skills

• Basic computer proficiency and EMR documentation experience

• Valid driver’s license, reliable vehicle, and acceptable driving record

Position Details:

• Direct Hire | Full-Time

• Schedule: Monday–Friday, Day Shift (flexible to patient needs)

• Pay Range: $33 – $46 per hour

• Mileage reimbursement: $0.70/mile

• Sign-On Bonus: $10,000
