Job Responsibilities:
- Perform Start of Care (SOC) and OASIS assessments
- Conduct recertification and resumption of care visits
- Provide wound care and supervisory visits
- Document visits in Homecare Homebase EMR
- Coordinate care with physicians and the care team
- Educate patients and caregivers on care plans
- Follow agency policies, state regulations, and infection control standards

Qualifications:
- Active and clean RN license in Texas
- Valid driver’s license
