Job Responsibilities:
- Conduct comprehensive assessments using OASIS for admission, recertification, resumption of care, and discharge planning.
- Develop and implement individualized care plans for patients.
- Perform supervisory visits to ensure quality care delivery.
- Collaborate with the healthcare team to coordinate patient care and provide updates on patient progress.
- Ensure compliance with all federal, state, and local regulations and guidelines.
- Maintain accurate and timely documentation of patient care and services provided.
- Manage a caseload of 15-25 visits per week, ensuring timely and effective delivery of care.
- Provide education and support to patients and their families regarding diagnosis, treatment plans, and ongoing care needs.

Qualifications:
- Current Registered Nurse (RN) license in the state of Texas.
- 1 year experience as an RN Case Manager in Home Health or Hospice.
- Proficiency in OASIS assessments and documentation.
- Valid driver's license and reliable transportation.
