Job Responsibilities:
- Conduct start-of-care assessments
- Complete OASIS assessments
- Perform recertification visits
- Conduct resumption-of-care visits
- Carry out wound care supervisory visits
- Educate patients and families while providing culturally sensitive care

Qualifications:
- Active RN license in Texas
- 1+ year of Home Health experience
- Strong communication and interpersonal skills
- Ability to educate patients and families effectively
- Commitment to providing compassionate, culturally sensitive care
