Introduction:
We are seeking a dedicated and skilled Registered Nurse (RN) to join our growing Home Health team. In this role, you will provide patient-centered care in a supportive environment, delivering high-quality healthcare directly in patients’ homes while making a meaningful impact on their well-being.

Position Offers:
- Monday–Friday, 8:00 AM – 5:00 PM

Key Responsibilities:
- Conduct start-of-care assessments
- Complete OASIS assessments
- Perform recertification visits
- Conduct resumption-of-care visits
- Provide wound care and supervisory visits
- Deliver patient and family education
- Document accurately and timely using EMR (Homecare Homebase)
- Provide compassionate, culturally sensitive, patient-focused care

Qualifications:
- Active RN license in the state of Texas
- Minimum of 1 year of home health experience
- Familiarity with EMR system Homecare Homebase
- Strong communication and clinical assessment skills
- Ability to work independently and manage time effectively
