Key Responsibilities:

Perform start of care visits, recertifications, and patient discharges in the home setting
Provide direct patient care using established nursing practices
Assess patient needs and develop individualized care plans
Administer medications and treatments as prescribed
Monitor and document patient conditions and progress
Collaborate with multidisciplinary teams to coordinate care
Educate patients and families on health maintenance and disease management
Maintain accurate and compliant medical records

Qualifications:

Active Registered Nurse (RN) license in California
Minimum of 1 year of experience as a Registered Nurse
Home health experience preferred
Strong clinical knowledge and patient care skills
Experience with electronic health record (EHR) systems
BLS certification required; ACLS or PALS preferred
