WellTech Partners is partnering with a five star high-quality, clinician-led home health organization in San Jose, CA to identify an experienced RN Case Manager who is passionate about patient-centered care and ready to grow into a clinical leadership role.
This organization is known for its concierge-style care model, prioritizing meaningful patient relationships, longer visit times, and high clinical standards — not volume-driven care.
About the Role
This is a hybrid clinical + leadership role designed for an RN who wants to:
Deliver high-quality care in the field
Manage and coordinate interdisciplinary patient care
- Grow into a clinical supervisory position over time
Target structure:
- ~70% field-based patient care
- ~30% care coordination and team oversight
- Opportunity to transition into more leadership as the organization grows
What You’ll Do
Patient Care & Case Management
- Manage a caseload of home health patients across the San Jose area
- Conduct patient assessments, including Start of Care (SOC) visits
- Develop and oversee individualized plans of care
- Deliver high-quality, relationship-driven patient care (not rushed visits)
- Monitor patient progress and adjust care plans as needed
Care Coordination & Team Collaboration
- Coordinate care across disciplines (RNs, LVNs, PTs, OTs, etc.)
- Serve as a central point of contact for patient care planning
- Communicate effectively with physicians, caregivers, and internal teams
- Ensure continuity of care and strong patient outcomes
Clinical Oversight & Leadership Growth
Support day-to-day clinical decision-making and team guidance
Act as a resource for clinical questions and escalations
- Help build and support a clinical supervisory layer within the organization
- Transition into increased oversight responsibilities as patient census grows
Documentation & Compliance
- Complete accurate and timely documentation
- Ensure compliance with Medicare guidelines and home health standards
- Utilize OASIS documentation in patient assessments
What We’re Looking For
Required:
- Active California RN license in good standing
Home health experience (strongly preferred)
- Experience with OASIS documentation (highly preferred)
- Ability to manage patient care independently
- Strong clinical judgment and decision-making skills
- Excellent communication and interpersonal skills
Preferred:
Experience coordinating interdisciplinary care teams
Prior exposure to leadership or supervisory responsibilities
- Desire to grow into a clinical leadership role
- Experience working with geriatric patient populations
Schedule & Expectations
Monday–Friday, standard business hours (typically 9–5)
Field-based role with travel throughout San Jose and surrounding areas
Flexibility to respond to occasional after-hours clinical questions as needed
- Balanced workload with emphasis on quality of care over volume
